Calf Augmentation with Latissimus Dorsi Muscle Free Flap in Car Tire Friction Burn Patient

Abstract
Purpose: Car tire friction burn can cause wide soft tissue defect or deficiency on the lower leg and foot. The lattisimus dorsi muscle can be used in coverage and augmentation of these defect and deficiency. But, there is the limitation in applying to the young female patient because of the donor site long scar. Then we report the calf reconstruction and augmentation method with minimal donor site scar.

Methods: Patient was 27 years old married woman whose right calf area had been injured at 20 years ago by car tire. We harvested lattismus dorsi muscle with minimal donor site scar by using of endoscope. One artery and two venae comitantes anastomoses was performed. After muscle flap inset was done, sheets of split thickness skin (8/1,000 inch) were harvested with Zimmer? dermatome, then we engraft this skin onto muscle flap. The hospitalized period was 2 weeks.

Results: Both surgeon and patient was satisfied. Her right lower leg was reconstructed functionally and aesthetically and restorated similarilly compared with opposite calf volume. She was pleased she could wear stocking.

Conclusion: Endoscopically harvested lattismus dorsi muscle free flap can be one of the choices in young woman patient with moderate to severe friction burn induced soft tissue deficiency in lower leg with minimal donor site scar. (Journal of Korean Burn Society 2008;11:50-53)
